Within 30 calendar days after admission, the ICF/IID shall identify an interdisciplinary team for each person supported. The interdisciplinary team must be composed of the person and the person's QDDP, the person's family, guardian, or advocate, if applicable, other ICF/IID staff, and any other individual chosen by the person. Meetings of the interdisciplinary team must be scheduled and conducted so they facilitate the active participation of all members of the team. The interdisciplinary team must meet at least annually; however, the person or any other member of the interdisciplinary team may request a team meeting at any time.
