The ICF/IID shall develop and maintain a record keeping system that includes a separate record for each person supported that documents the person's health care, active treatment, social information, and protection of the person's rights. The ICF/IID shall provide electronically-stored records with security to prevent access by unauthorized personnel. The ICF/IID shall keep confidential all information contained in the person's records, regardless of the form or storage method of the records. Policies and procedures governing the release of any information, including consents necessary from the person supported, a parent if the person is under 18 years of age, or the legal guardian, if any, must be written and executed.
