If a participant receiving services under this chapter transfers from one provider to another provider, the transferring entity shall deliver a copy of the participant's record to the new entity upon obtaining consent by the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian, if any. A copy shall be made available to the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian, if any.
