The participant or the participant's parent or guardian shall be provided with the following:

1. (1) A list of providers;
2. (2) A list of OHCDS qualified providers;
3. (3) A list of HCBS waiver services;
4. (4) Information regarding the appeal process; and
5. (5) Provided with contact information to request a fair hearing upon initiation of services and annually thereafter.
