The ISP team meeting shall be scheduled and conducted in a manner which facilitates the active participation of all team members. The case manager shall encourage the participant to choose the location of the meeting and shall document how the participant was involved in the planning and facilitation of the ISP meeting and the plan development. If the participant is unable or unwilling to participate in the meeting, the case manager shall document the source of input on the participant's behalf.
