The following individuals shall have the opportunity to participate in the development of the ISP:

1. (1) The participant;
2. (2) The participant's parent if the participant is under 18 years of age;
3. (3) The participant's guardian, advocate, and/or conservator, if any;
4. (4) The participant's case manager;
5. (5) Direct HCB services staff; and
6. (6) Any other individual desired by the participant.
