A provider must maintain records for six years or supervise the maintenance of records, including:

1. (1) Any applications for a person that was accepted and is receiving services including any supporting documents;
2. (2) Any documents that determine eligibility;
3. (3) Any documents pertaining to the provision of services;
4. (4) Any documents on administrative costs;

5. (5) Any fiscal documentation or other records, and information necessary for reporting and accountability as required by the division.

   If a participant changes direct home and community-based (HCB) service providers, the provider shall transfer the participant's records to the new provider of HCB services. Each provider must have a written procedure for destruction of records.
