The provider shall obtain consent prior to releasing a participant's confidential information. The consent shall:

1. (1) Be time limited;
2. (2) Specify the information to be released;
3. (3) Specify to whom the information will be released; and
4. (4) Be signed by the participant, if applicable, or by the participant's parent if the participant is under 18 years of age, and the participant's guardian, if any.
