44 : 81 : 02 : 01 . Immunization requirements. Any full time or p a r t time student entering an early childhood program in South Dakota shall present certification that the student has received the following immunizations:

1. (1) Four or more doses of DTaP, with at least one dose administered on or after age four. A child receiving six doses before age four does not require any additional doses for school requirements;
2. (2) Four or more doses of poliovirus vaccine, at least one dose on or after age four;
3. (3) Two doses of MMR vaccine or the submission of serological evidence of immunity. Minimum age for the first dose is twelve months with the second dose routinely administered between the ages of four and six. The second dose may be administered prior to age four provided at least twenty-eight days have elapsed since the first dose; and

4. (4) Two doses of varicella vaccine. The minimum age for the first dose of varicella vaccine is twelve months. History of disease is acceptable in lieu of vaccination with parent or guardian signature.

   A child under the age of four only requires three DTaP, three Polio, one MMR, and one varicella vaccine. At age four the above rules apply.
