Each registrant shall pay a registration fee at the time of initial registration or at renewal of registration. Registration fees are non-refundable and may not be prorated. The registration fees are as follows:

1. (1) $150 for any dentist, medical doctor, optometrist, osteopathic doctor, pharmacy, veterinarian, or podiatrist;
2. (2) $150 for any nurse practitioner, nurse midwife, or physician assistant;
3. (3) $75 for any manufacturer, distributor, analytical lab, euthanasia, teaching institution, or researcher (includinganydrugdetectiondogtrainer); and
4. (4) $75 for any locum tenens certificate.
