Emergency medical service personnel shall recognize that a person has executed an EMS cardiopulmonary resuscitation directive pursuant to this chapter if:

1. (1) The personnel are presented with the standard form completed pursuant to this chapter; or
2. (2) The person is wearing a bracelet issued as provided by this chapter.
