If a person wishes to direct emergency medical service personnel to withhold resuscitation attempts when the person is in cardiopulmonary arrest, the person may complete a standard form for an EMS cardiopulmonary resuscitation directive as provided by this chapter. The department shall provide the standard form to physicians, physician assistants, and nurse practitioners.
