A person or organization that desires to operate an ambulance service must obtain a ground or air ambulance license from the department. The person or organization must submit a completed application to the department at least thirty days before the expected date of operation when applying for a new license. A person or organization must submit a completed renewal application to the department thirty days before the expiration of an existing license. A fee of twelve dollars must accompany the application.
