The department shall develop and implement an intraagency agreement between the state special education program and Part C program that includes the following:

1. (1) Notification procedures to the state special education department and appropriate school district;
2. (2) Transition conference to discuss services and meeting to develop transition plans; and
3. (3) Applicability of transition requirements under Part C and Part B pursuant to this article and article 24:05.
