Nutrition services include the following:

1. (1) Conducting individual assessments in the following:

   1. (a) Nutritional history and dietary intake;
   2. (b) Anthropometric, biochemical, and clinical variables;
   3. (c) Feeding skills and feeding problems; and
   4. (d) Food habits and food preferences;
2. (2) Developing and monitoring plans to address the nutritional needs of children eligible under this article, based on the findings resulting from individual assessment; and
3. (3) Making referrals to community resources to carry out nutrition goals.
