The annual registration and license fees for participants, in any capacity, in a boxing, kickboxing, or mixed martial arts contest are as follows:

1. (1) Judge, $50;
2. (2) Boxer, $50;
3. (3) Kickboxer, $50;
4. (4) Mixed Martial Artist, $50;
5. (5) Matchmaker, $200;
6. (6) Manager, $100;
7. (7) Promoter, $300;
8. (8) Referee, $50;
9. (9) Second, $25;
10. (10) Timekeeper, $25; and

11. (11) Physician, no charge.

    An amateur organization or educational institution applying for an exemption shall pay a fee of $100.
