An application for the following must be made in writing on a form prescribed by the board:
(6) Reactivated license or certificate.
The application, license, or certification fee must be submitted with the application. The application fee is non-refundable.
The applicant shall verify under penalty of perjury that all information on the application is true and correct. The electronic submission of an application serves as the applicant’s signature and verification of the information on the application. The board may require the applicant clarify information on the application or provide additional information.
An application is not complete until the board receives the required documentation and fees. An application that remains incomplete one year after the initial submission of the application is considered to be withdrawn.
