The board shall collect in advance the following nonrefundable fees from applicants:

1. (1) For initial licensure or endorsement from another state, $100;
2. (2) For issuing a temporary permit, $25;
3. (3) For biennial renewal of a license, $95;
4. (4) For reinstatement of a lapsed license, the current renewal fee plus $50;
5. (5) For providing a transcript, $5;
6. (6) For endorsement to another state, territory, or foreign country, $25;
7. (7) For effecting a name change on the records of the license holder, $10;
8. (8) For issuance of a duplicate license, $20; and
9. (9) For placing the license on inactive status, $10.
