An individual who requests and receives a prescription drug or medical supply from the program shall, prior to receipt of the drug or medical supply, sign an acceptance form attesting that:

1. (1) The individual is an eligible patient;
2. (2) The individual acknowledges that the drug or supply has been donated; and
3. (3) The individual waives the requirement for child-resistant packaging, if applicable.
