The absence of a record signifies that counseling was accepted and provided or that an offer was made. Failure to complete counseling to a patient or caregiver shall be recorded for the following instances:

1. (1) The patient or caregiver refuses to accept the pharmacist's personal oral counseling;
2. (2) Counseling was not practicable; or
3. (3) Counseling could not be accomplished by telephone contact.
