The pharmacist or intern receiving the transferred prescription information shall:

1. (1) Write the word "transfer" on the face of the transferred prescription;

2. (2) Record on the transferred prescription:

   1. (a) The original date of issuance;
   2. (b) The original prescription number and the number of refills authorized on the original prescription;
   3. (c) The number of valid refills remaining and the date of the last refill;
   4. (d) The name and address of the pharmacy from which the prescription information is transferred; and
   5. (e) The name of the transferring and the receiving pharmacist or intern; and
3. (3) Clarify verbally any unclear information on a facsimile.
