The pharmacist or intern transferring the prescription information shall:

1. (1) Record on the original prescription:

   1. (a) The name and address of the pharmacy to which the prescription is transferred;
   2. (b) The name of the pharmacist or intern receiving the prescription information;
   3. (c) The name of the pharmacist or intern transferring the prescription information; and
   4. (d) The date of the transfer.
2. (2) Record the number of refills transferred. If all refills are transferred, the original prescription shall be marked "void".
