Application for pharmacy license--Annual renewal required. A pharmacist operating a pharmacy in this state shall apply, on forms provided by the board, each year to the board for a license to operate the pharmacy. The fee for initial licensure is two hundred dollars and the fee for license renewal is two hundred dollars.
