An applicant for licensure by examination shall provide the following to the board with the application:

1. (1) The application fee of thirty-five dollars;
2. (2) A photo of the applicant;
3. (3) A transcript showing graduation from a college of pharmacy approved by the American Council on Pharmaceutical Education; and
4. (4) A government-issued form of photo identification.
