A nurse aide training program shall maintain the following documentation, and provide it to the board upon request:
- (1) A list of students enrolled in the program;
- (2) The date a student completed the program, failed the program, or withdrew from the program, and the reason for withdrawing from the program; and
- (3) A list of the individuals teaching the program, including qualifications and experience.
