Hospitals requesting approval shall submit an application to the board, on forms provided by the board, furnishing the following information:

1. (1) The name of the hospital;
2. (2) The address of the hospital;
3. (3) A list of medical or osteopathic postgraduate programs;
4. (4) The number of physicians enrolled in postgraduate programs;
5. (5) Affiliation with other institutions, if any; and

6. (6) Any other information requested by the board.

   A hospital once approved shall continue its approved status until further action is taken by the board.
