The applicant shall provide information on any experience or training in a hospital or care of the sick, the name of any state which previously issued a chiropractic license to the applicant, the length of time in practice, the location of practice, whether the license is in force or has been suspended, revoked, or voluntarily discontinued, and the date of issue and certificate number of any national board certificate.
