Duplicate certificate fees. Duplicate certificate fees are as follows:

1. (1) Intern, five dollars;
2. (2) Licensee, fifteen dollars; and

3. (3) Business entity, fifteen dollars.

   These fees may not apply to any duplicate license obtained electronically by the licensee directly from the licensee's profile on the board's database.
