Each licensee or sports wagering services provider shall maintain a record of each tournament, contest or pool it offers that must include all of the following:

1. (1) Name or identification of the tournament, contest, or pool;
2. (2) The date and time the tournament, contest, or pool occurred or will occur;
3. (3) Events and wager types;
4. (4) Rules concerning tournament, contest, or pool play and participation;

5. (5) Each registered patrons:

   1. (A) Unique patron ID and username;
   2. (B) Amount of entry fee collected, including any bonus or promotional credits, and the date collected;
   3. (C) Scorings and rankings; and
   4. (D) Amount of winnings paid, including any bonus or promotional credits, and the date paid;
6. (6) Total amount of entry fees collected, including any bonus or promotional credits;
7. (7) Total amount of winnings paid to patrons, including any bonus or promotional credits;
8. (8) Total rake, commission, or fees collected;
9. (9) Funding source amount or amounts comprising the prize pool;
10. (10) Prize structure on payout;
11. (11) Methodology for determining winner or winners; and
12. (12) The current status of the tournament, contest, or pool.
