If available, the following information and data shall be provided for each excluded person:

1. (1) The full name and any aliases the person is believed to have used;
2. (2) A description of the person's physical appearance, including height, weight, type of build, color of hair and eyes, and any other known physical characteristics which may assist in the identification of the person;
3. (3) Date of birth;
4. (4) The effective date the person's name was placed on the list; and
5. (5) A photograph of the person and the date thereof.
