Any person seeking approval of a new game or a variation of a game of poker, blackjack, craps, roulette, keno or blackjack-poker combination shall submit an application on a form provided by the commission and pay the fee required by § 20:18:06:02. The application shall include information concerning the following:

1. (1) Personal background information;
2. (2) Proposed rules of the game;
3. (3) Approval of the game in other jurisdictions; and
4. (4) The name of the operator or tribal casino which will offer the game for play if approved.
