Any person who desires to supervise a registered trainee appraiser under this article must apply in writing for approval as a supervisory appraiser on a form provided by the secretary. The nonrefundable application fee prescribed in § 20:14:10:01 must accompany the application form. The application must contain the following:

1. (1) Name;
2. (2) Driver's license number;
3. (3) Social security number;
4. (4) Home and business addresses;
5. (5) Home and business telephone numbers;
6. (6) Business name where employed;
7. (7) Area of geographic competency;

8. (8) Certification of competency in one or more of the following appraisal assignments:

   1. (a) Residential one-to four family;

   2. (b) Nonresidential:

      1. (i) Commercial;
      2. (ii) Industrial;
      3. (iii) Agricultural; or
      4. (iv) Multifamily; and
9. (9) Email address.
