The utility shall publish customer information which shall be provided to a customer upon request at no charge. The customer information shall be submitted to the commission for approval. A list of the available customer information publications and the customer's applicable rate schedule shall be furnished to each new customer. The customer information must, at a minimum, include the following:

1. (1) The utility's own customer rules;
2. (2) The title, address, and telephone numbers of the utility department or departments from which information can be secured or complaints filed, including a telephone number which customers may call in emergency situations;
3. (3) The deposit or guarantee requirements of the utility;

4. (4) A notice of the commission's regulation of the utility and the customer's right to appeal a utility's decision to the commission.

   Existing customers shall be furnished with a copy of any changes in tariffs applicable to their service.
