The notice of disconnection sent to a subscriber shall contain a specific explanation of the following:

1. (1) The particular ground upon which the proposed disconnection is based;
2. (2) The telecommunications company's intention to disconnect service unless the subscriber takes corrective action;
3. (3) The corrective action which the customer must take to avoid disconnection.
