Each telecommunications company holding a deposit shall keep a record of the deposit until the deposit is refunded. The record shall show:

1. (1) The name and current billing address of each depositor;
2. (2) The amount and date of the deposit;

3. (3) Each transaction concerning the deposit.

   The company shall provide each depositor with such information upon termination of the depositor's service.
