All insurers or persons subject to SDCL chapter 58-5A, except as provided in SDCL 58-5A-20, shall file the following forms with the director:

1. (1) Form A, statement regarding the acquisition of control of or merger with a domestic insurer, pursuant to SDCL 58-5A-4;
2. (2) Form B, insurance holding company system annual registration statement, pursuant to SDCL 58-5A-21;
3. (3) Form C, summary of changes to the previous year's registration statement, pursuant to SDCL 58-5A-21 or, changes to the current year's registration statement, pursuant to SDCL 58-5A-24;
4. (4) Form D, prior notice of a transaction, pursuant to SDCL 58-5A-56;
5. (5) Form E, pre-acquisition notification, pursuant to SDCL 58-5A-45; and

6. (6) Form F, enterprise risk report, pursuant to SDCL 58-5A-29.1.
