To establish an account with a multi-jurisdictional totalizator hub an application form must be signed or otherwise authorized in a manner approved by the commission and include the following information about the applicant:

1. (1) Full legal name;
2. (2) Principal residence physical address;
3. (3) Principal residence mailing address, if different than physical address
4. (4) Telephone number;
5. (5) Social security number; and
6. (6) Proper identification or certification demonstrating that the applicant for an account is at least 18 years of age, if a resident of South Dakota, or 21 years of age if not a South Dakota resident.
