An applicant for any of the required satellite licenses must apply on a form provided by the commission. The application may include questions concerning the following:

1. (1) Personal background information;
2. (2) Financial information;
3. (3) Participation in legal or illegal gaming activities in South Dakota or other jurisdictions, including foreign countries; and
4. (4) Other information as required.
