All cervidae entering South Dakota must be accompanied by a certificate of veterinary inspection and an entry permit number requested by the licensed, accredited veterinarian signing the certificate of veterinary inspection and issued by the state veterinarian before movement. One of the following statements must appear on the certificate of veterinary inspection:

1. (1) "All cervidae identified on this certificate of veterinary inspection originate from a herd in which all cervidae have been kept for at least three years or into which they were born. There have been no exposure to or additions from any other source in the past three years. There have been no diagnosis, signs, or epidemiologic evidence of CWD in this herd for the past three years. Records and causes of death for the past three years in this herd of origin are available to the animal health official of the state of origin"; or
2. (2) "All cervidae identified on this certificate of veterinary inspection originate from a herd which has been determined to have the stated monitored status by the animal health official of the State of South Dakota"; or

3. (3) "All cervidae identified on this certificate of veterinary inspection originate from a herd which has been determined to have certified CWD cervid herd status by the animal health official of the State of South Dakota."

   All certificates of veterinary inspection also must have the following statement with the signature of the cervid owner attesting validity. "No animal has ever originated from, or been a member of a herd, where CWD has been diagnosed, or been a member of a CWD traceback or traceforward herd in the past five years."
