Each licensed pesticide dealer must keep records of restricted use pesticide sales. The record for each sale must include:

1. (1) The dealer's name, address, and license number;
2. (2) The private or commercial applicator's name, address, certification or license number, the expiration date of the private or commercial applicator's certification or license, and the applicator's certification categories relevant to the pesticide sold;
3. (3) The date of the sale; and

4. (4) The trade name, quantity, and Environmental Protection Agency registration number of the pesticide sold, including any applicable emergency exemption or state special local need registration number.

   Records required by this section must be maintained in the form of a log, registry of invoices, billing tickets, or delivery tickets.
