Growers subject to the assessment may request a checkoff fee refund application from the council. The application must be on forms provided by the council and must contain the following information:

1. (1) The date of the application;
2. (2) The name, address, and signature of the grower;
3. (3) The number of bushels of soybeans for which a fee was paid;
4. (4) The total dollar amount of the fee paid;
5. (5) The date the fee was paid; and

6. (6) The name and address of the first purchaser who collected the fee.

   The refund application must be accompanied by a copy of the record of assessment.
