The VALU loan application form shall include the following:

1. (1) The requested amount of the loan and guaranty;
2. (2) The purpose;

3. (3) Additional documentation, including the following information:

   1. (a) A cash flow budget;
   2. (b) A signed financial statement completed no more than 30 days prior to application;
   3. (c) A repayment plan;
   4. (d) Tax forms for the two most recent years, including 1040 and 1040F, if applicable;
4. (4) The lender's signature and the date of signature;
5. (5) The applicant's signature and the date of signature.
