The application for certificate of authorization to provide postsecondary education, as prescribed by the Secretary of State, shall contain the following fields:

1. (1) Name of institution;
2. (2) Address of institution;
3. (3) Contact person for institution;
4. (4) Addresses of other locations where institution provides classes;
5. (5) Accrediting information; and
6. (6) Institution classification.
