The organizational petition for formation of a county road, ambulance, rural fire protection, sanitary, watershed, or water project district must be in the following form:

1. (1) Road districts insert:

   1. (a) The need for road work in the territory described; and
   2. (b) Request county commissioners to define boundaries for the district and make a determination that such district be created;
2. (2) Ambulance districts insert area in square miles to be included in district;
3. (3) Fire districts insert area in square miles to be included in district;

4. (4) Watershed districts insert:

   1. (a) The need in the interest of the public health, safety, and welfare for creation of a district to accomplish improvements in the watershed;
   2. (b) Statement of the purposes of contemplated improvements;
   3. (c) List of landowners and total acreage owned by each;
   4. (d) Map of district and land ownership;
   5. (e) Location of the official place of business of the district; and
   6. (f) The number of managers; and

5. (5) Water project districts insert:

   1. (a) The object and purpose of the district and the works proposed to be constructed with a description of the nature, location, and method of operation;
   2. (b) Location of the principal place of business of the district; and

   3. (c) The number of board members.

      The balance of this petition form is prescribed in § 5:02:08:00.03.

WE, THE UNDERSIGNED, are qualified voters in the area herein described. We petition that the question of forming the __________________ (insertnameandtypeofdistrict) district be submitted for a vote of approval or rejection pursuant to law.
