The tax levy opt-out ballot must be white and in the following form:

1. (ElectionDate) The following tax levy opt-out was passed by the governing body and (submitted) (referredbypetition) to the voters. The tax levy opt-out does not become effective unless approved by majority vote.

2. (HERELISTTHEAMOUNTANDTERMSOFTHEINCREASEDTAXLEVY) To vote use a cross (X) or check mark (ü) in the square in front of "Yes" or "No".

   □ Yes A vote "Yes" will increase the tax levy by the amount of the opt-out.

   □ No A vote "No" will leave the tax levy as it is without the amount of the opt-out.
