2 : 01 : 02 : 04.02 . Notice of discontinuance of employment of law enforcement officer . A law enforcement agency shall submit a completed law enforcement resignation or termination form to the commission within ten days after a law enforcement officer ceases employment for any reason with that law enforcement agency .
