1. 1. Each application for a construction permit for new installation, alteration, or relocation shall be made in a manner as required by the Director and shall include three (3) copies of:

   1. A. Detailed plans including:

      1. (1) Sectional plan of car and hoistway;
      2. (2) Sectional plan of machine room;
      3. (3) Sectional elevation of hoistway and machine room, including the pit, bottom and top clearance of car, and counterweight;
      4. (4) Size and weight of guide rails, and guide rail bracket spacing.
   2. B. Name and address of the person who designed the installation for which plans are submitted; and
   3. C. Statement of Contract Price.

2. 2. Each application for a facility registration shall be made in a manner as required by the Director and shall include the following for each facility:

   1. A. Name and address of the owner;
   2. B. Location;
   3. C. Manufacturer;
   4. D. Model or Type;
   5. E. Contract load and speed;
   6. F. Purpose or use;
   7. G. Date of installation; and
   8. H. Number of floors.
