1. A. The employer’s representative shall file a Form 18, Periodic Report, or the EDI equivalent Sub Annual (SA), as follows:

   1. (1) Six months after the alleged date of injury and each six months thereafter until the Commission’s file is closed; and
   2. (2) At the request of the Commission.
