1. A. The employer’s representative shall report to the Commission injuries reported by the employer pursuant to R.67-411C(1).
2. B. This report shall be made in accordance with R.67-416 within ten days of closing by the employer’s representative.
3. C. Late reports shall be subject to a fine for late reporting plus an additional penalty of five dollars for each day late.
