1. A. Serve the following parties:

   1. (1) The insurer’s designated recipient as in R.67-401 (the employer is not served);
   2. (2) The employer directly when the employer is uninsured;
   3. (3) The party when the party is not represented by an attorney;
   4. (4) The Second Injury Fund;
   5. (5) The State Accident Fund.
2. B. When an attorney represents a party, and notifies the Commission of his or her representation, the attorney is served. The party is not served.
